21-Day Habit & Identity Journal

Welcome to your 21-day program for building lasting change. This journal is a tool to help you apply the principles of the Fogg Behavior Model and the Identity Affirmations you’ve been working with. Over the next three weeks, you’ll focus on a single tiny habit and use it as a foundation for rewiring your mindset and identity.

Each day, you will be prompted to:

  1. Identify one tiny habit that you will commit to for the day. (Remember: something that takes less than 30 seconds and is ridiculously easy.)

  2. Reflect on a core concept or affirmation.

  3. Answer a meta model question to tap into the subconscious mind.

Week 1: Foundations of Behavior
Week 1: Foundations of Behavior Focus: Understanding and applying the Fogg Behavior Model (B=MAP). Your goal this week is to consistently perform one tiny habit, no matter how small, and to celebrate each success.

  • Day 5: Navigating Motivation
  • Day 6: Consistency Over Perfection
  • Day 7: Weekly Review

Week 2: Identity & Reframing
Week 2: Identity & Reframing Focus: Connecting your new tiny habits to the identity affirmations. Your goal is to see your tiny habit not just as an action, but as an expression of who you are becoming.

Week 3: Visionary Living
Week 3: Visionary Living Focus: Living from your new, empowered identity. Your goal is to recognize and act from a place of truth, alignment, and courage.
